Description

Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. Strafford's community Registered Nurse will coordinate the delivery of needed services, provide assistance/education around following medical advice and proper medication taking, assess in-home mobility safety to reduce the risk of falls, assist families with understanding the issues and needs of the family member requiring in-home care services, and provide ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.
