Description

Provides professional assistance with the assessment and development of plans for the care of individuals who, because of age, illness, disability or other difficulties, need assistance in planning and arranging for in-home care services. The Sharon Health Initiative's community health care worker will coordinate the delivery of needed services like adequate food and shelter and overall management of th individual's health well-being. This may include provision of assistance/education around following medical advice and proper medication taking, assessing in-home mobility safety to reduce the risk of falls, assisting families with understanding the issues and needs of the family member requiring in-home care services, and providing ongoing follow-up visits to ensure the best possible health outcomes. All services provided are confidential and specific to each individual's needs.
