Provides assistance in navigating Vermont Health Connect, Green Mountain Care and FQHC health care systems to aid clients in obtaining free or subsidized health care through referrals, including: benefit screening and enrollment assistance, dental referrals and assistance in obtaining care, patient assistance and pharmacy programs, eye care referrals, tobacco use screening and cessation program referrals, and physician referrals.

Provides holistic care services to all community members that may include the following: Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Community Connections Community Health Team also provides Resource Advocates who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. The team also includes VT Health Connect navigators and assistors, as well as care managers for individuals who require support maintaining independence and stability within the community.

Provides support to individuals and families in the form of resources or referral services for basic needs, family crises, mental health issues, parenting issues, and child care. Offers assistance by a certified application counselor with enrollment in health care insurance. Assists individuals in applying for 3SquaresVT, subsidized housing, and fuel assistance.

Provides information and guidance to consumers about all types of commercial insurance. Addresses questions and concerns, including the appeal of health insurance claim denials. Provides information about insurance rates and products, state insurance laws, licensed Vermont companies, and individual agents.

Grace Cottage's Community Health Team also provides a Resource Advocate who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities.
