Helps residents apply for 3SquaresVT, School Breakfast and Lunch, WIC and other food assistance programs. Offers nutrition education and activities to persons eligible for 3SquaresVT. Acts as advocate for food stamp issues. Distributes Farm to Family coupons and Senior Farmers Market coupons, which can be used to purchase fresh fruit and vegetables at participating farmers markets in Vermont.

Provides community food shelf services. Personal care items may be available depending on donations. Provides access to USDA surplus commodity foods. May distribute Thanksgiving turkeys. All participants seeking food services are screened for possible USDA food assistance programs and 3SquaresVT eligibility. Delivery available to eligible individuals.

Assists individuals in completing applications for benefits, including housing applications, child care tuition subsidy application, health care application, disability applications, and application for financial assistance from Vemont Division of Economic Services. Assists with attempts to access local and state emergency financial assistance when applicable. Personal budgeting counseling sessions are offered and made available to all individuals seeking financial assistance.

Provides assistance in navigating Vermont Health Connect, Green Mountain Care and FQHC health care systems to aid clients in obtaining free or subsidized health care through referrals, including: benefit screening and enrollment assistance, dental referrals and assistance in obtaining care, patient assistance and pharmacy programs, eye care referrals, tobacco use screening and cessation program referrals, and physician referrals.

Provides preventative health care education and referrals for individuals and families. Services include assistance with accessing benefit programs, assistance finding housing and employment, health education and wellness programs, and referrals to health care and to local resources such as volunteer drivers, utility assistance, and food assistance programs. Also provides meeting space for social services agency use and health and wellness programs.

Provides holistic care services to all community members that may include the following: Nursing, Behavioral Health and Physical Health Coaching services: Nutrition education, exercise education and counseling, short-term behavioral counseling, Diabetes education, individual health coaching, medication management, weight management-individual and group support, smoking cessation, Advanced Directives, Fall Safety assessments, medical transportation and dental solutions. Community Connections Community Health Team also provides Resource Advocates who will assist with completing applications for the following services and assistance: health insurance (VT Health Connect and Medicare and Medicare D comparisons), SSDI/SSI, and prescription costs, as well as application assistance for housing, food, and utilities. The team also includes VT Health Connect navigators and assistors, as well as care managers for individuals who require support maintaining independence and stability within the community.

Provides low-cost or free health care for adults. Vermont Medicaid includes doctor visits, hospital care, prescriptions, vision, and some dental care. Also covers long-term care in a nursing home or at home, and physical therapy. Medicaid for the Aged, Blind, or Disabled (MABD) is also available. Assists Vermonters in enrolling in (and answers questions about) Medicaid and MABD. The member services line can help with filling out the application.
