Operates a resource contact center that connects children and their families to services they need. HMG Care Coordinators provide education and support to families around developmental or behavioral concerns, help families recognize typical developmental milestones, offer confidential developmental screening, provide referrals to community-based supports, empower families to overcome barriers to services, and follow up with them to make sure linkages are successful. Help Me Grow also provides referrals to health and human services, including therapists.

Provides a perinatal home visiting program that increases expectant and new parents' knowledge of and access to supports and services for their families. HEART home visitors are able to offer an array of support services, access to concrete supports, and referrals to additional programming and services. This program provides counseling, in the home, or via telemental health (online), or the office setting for mothers. For families who need a higher level of care, home visitors are able to refer families to their HEART home visiting perinatal mental health clinicians. The HEART In-Home Counseling program clinicians can address a variety of symptoms including depression, anxiety, OCD, and PTSD following a traumatic birth experience.

Convenes a multi-disciplinary group of service providers that meets monthly to coordinate services for pregnant and postpartum women with opioid use disorder and their infants. Encourages mothers to stay with their substance abuse treatment, get good prenatal care, and learn how to safely mother their newborns.

Provides provides information on the safety of medications and other exposures during pregnancy and while breastfeeding. Information Specialists answer questions about the safety and risk of exposures, such as medications, vaccines, chemicals, herbal products, substances of abuse, maternal health conditions and more, during pregnancy or breastfeeding.

Provides free or low-cost health insurance for immigrants. Covers hospital, medical, dental, and prescription drug costs.

Offers a free, 10-session educational support group that serves pregnant and parenting women who are at risk for alcohol or drug misuse, or who are experiencing the effects of a partners' substance misuse. The group helps women understand the risks associated with substance use behavior and introduces strategies to overcome the challenges of substance use and raising a family. The program is designed to provide knowledge, build skills, and offer a safe and caring climate for pregnant and parenting women. Child care and meals are provided during program sessions.

Provides low-cost or free health insurance for children and pregnant people. Benefits include doctor visits, emergency services, prescription medicines, dental care, hospital visits, vision care, mental health care, substance use disorder treatment, immunizations, and services for pregnant women such as lab work and tests and prenatal vitamins.
