Provides a full range of primary and preventive health care with an expertise in the special health care needs of transitioning people. Offers primary care for transgender patients, including referrals and preparation for gender confirming surgeries; Hormone therapy, including initiation and maintenance; Routine gynecological care; and Sexually Transmitted Infection (STI) screening. Also provides behavioral health services. The Community Health Pharmacy program offers discounted prescriptions to all patients. Also provides consultations and support for eligibility for State and Federal Benefits and low-cost drug manufacturer discount programs.

Provides a health screening program that pays for breast cancer screenings (mammogram's), cervical cancer screenings (Pap and HPV tests), as well as diagnostic follow up (breast ultrasounds, breast biopsies, colposcopies). Cholesterol and diabetes screenings are also provided. Participants can go to their own physicians or get help finding a primary care provider. Includes assistance with making healthy lifestyle changes related to eating, physical activity, and smoking.

Offers a hotline staffed by volunteers who are all trans-identified and educated in the range of difficulties transgender people experience. While their goal is to prevent self-harm, they welcome the call of any transgender person in need, including people who may be struggling with their gender identity and are not sure whether they are transgender. Operators call emergency services only with the caller's expressed consent.

Provide patients and families with comprehensive education and resources regarding available options for gender affirming care.

Offers a social and support group for individuals who are trans, non-binary, gender non-conforming, or questioning their gender identity and looking for a community with others who can relate.
